Entry Form Program - Step #2

Now that you have downloaded and saved the files start Microsoft Excel and open my_dog_list.xls. I have some sample data in here so you should change the information to your dog's information. The names at the very top (the fields / header) must remain exactly the same as they are. They are:

Stake - The stake that this dog will run in
Fee - The fee for that stake
Dog Name - The registered name
Call Name - The call name

For AKC registered dogs:

AKCchk - put an X here is you have an AKC number
AKC - the AKC registration number for the dog

This next part is for puppies or young dogs not yet registered. Leave these blank if you have an adult registration on the dog.

AKCLchk - put an X here is you have an AKC litter number
AKCLitter - the AKC litterregistration number for the dog

FDSB - The Field Dog Stud Book number if you have one
Reg Counrty - Country of registration
Breed - Your dog's breed (Brittany?)
DOB - Date of Birth
Sex - M or F
Sire - Sire's registered name
Dam - Dam's registered name
Breeder - Breeder's Name
Owner(s) - Owner's name
Address - Owner's address
City - Owner's address city
State - Owner's address state
Zip - Owner's address zipcode
Handler - Handler's Name
Address - Handler's address
City - Handler's address city
State - Handler's address state
Zip - Handler's address zipcode
Phone - Handler's phone number
e-mail - Handler's e-mail

These are the fields that should be filled out on any entry form. Save this file once you have done this to it's original name. Also save it to another file name like MASTER.XLS. In the future you will edit the MASTER.XLS and then save it as my_dog_list.xls for the next trial
